Warren Death Certificate Offices

Warren has two offices that deal with death records. The Trumbull County Combined Health District is at 176 Chestnut Avenue NE, Warren, OH 44483. Their phone number is 330-675-2489. The Warren City Health Department is at 418 South Main Street, Warren, OH 44481. Call them at 330-841-2541.

Both offices serve the Warren area but cover slightly different jurisdictions. The county health district handles deaths outside Warren city limits, while the city health department covers deaths within city boundaries. If you are not sure which office to call, start with the county office at 176 Chestnut Avenue. They can point you to the right place.

One thing to note: if the person died in the Girard city limits or at North Side Hospital, those records may not be available at the Trumbull County Health Department. You would need to check with the appropriate local registrar instead.

Getting Warren Death Certificates

There are several ways to get a Warren death certificate. Each method has a different timeline.

In person is the quickest option. Visit the Trumbull County Combined Health District at 176 Chestnut Avenue NE or the Warren City Health Department at 418 South Main Street during business hours. Bring the full name of the deceased and the date of death. The staff will search their records and print a certified copy. Payment is due at the time of the request. Cash, checks, money orders, and credit or debit cards are all accepted. Card payments may have a small processing fee.

By mail, send a completed request with your payment. Include a self-addressed stamped envelope. Make your check or money order payable to the appropriate office. Some offices do not accept personal checks for mail orders, so a money order is the safest bet. Mail your request to the Trumbull County Combined Health District, 176 Chestnut Avenue NE, Warren, OH 44483. Allow one to two weeks for processing and return mail.

Online, you can use VitalChek or the Ohio Department of Health website. Both add service charges. Under Ohio Revised Code Section 3705.23, death records are public, so you can order from any authorized source.

Warren Death Certificate Fees

A certified copy of a Warren death certificate costs $25.00 for the first copy. Each extra copy ordered at the same time is $10.00. This discount only applies when you order multiple copies in the same request.

The state fee through the Ohio Department of Health is $21.50 per certified copy as of January 2025. VitalChek adds a service charge that varies by shipping method. Section 3705.24 of the Ohio Revised Code sets the fee rules. Part of the fee stays with the local registrar and the rest goes to the state. This is why local and state fees differ.

Note: Death certificates are filed in the county where the death took place, not where the person lived, so confirm the death occurred in Trumbull County before ordering.

Warren Death Records Information

A Warren death certificate lists the deceased's full name, date of birth, date of death, place of death, and cause of death. It also includes the parents' names, marital status, usual address, and social security number. The funeral director gathers the personal facts. The doctor or coroner fills in the medical cause. Under Section 3705.16, every death must be registered with the local registrar.

For deaths in the past five years, the social security number is redacted. Only certain people can get a copy with the number showing. That list includes spouses, direct descendants, executors, attorneys, funeral directors, government investigators, private investigators, and veteran's service officers. You must prove your status to get the full version.

The Trumbull County Archives at 118 High Street NW, Warren, OH 44481 holds birth and death records from 1867 through 1908. They also have marriage records from the early 1800s. Uncertified copies cost 10 cents per page plus a self-addressed stamped envelope. These older records are ledger entries, not printed certificates like the modern ones.

The Trumbull County Probate Court also has historical vital records from 1867 through 1908. Contact them through the county system for certified copies of pre-1908 death records. Genealogical research requests are handled as time allows.

For statewide historical searches, the Ohio History Connection holds death records from 1908 through 1953. Their online Death Record Index covers 1913 to 1944 and 1954 to 1970. The Ohio Department of Health at 4200 Surface Road in Columbus keeps records from 1964 forward. Call 614-466-2531 for the state office.

Warren Death Records and Ohio Law

Ohio treats death certificates as public records. Section 149.43 of the Ohio Revised Code says public offices must make records available to anyone who asks. No family relationship is needed. No reason is required.

Section 3705.07 requires the local registrar to number each death certificate, sign it, keep a copy, and send the original to the state. This creates a backup for every Warren death record. Section 3705.29 makes it a crime to file a false death certificate or alter one after it has been filed. These rules apply to all death records filed in Warren and the rest of Trumbull County.
