You do not need to be a relative or give a reason for your request.
Mentor Death Records and Privacy Rules
Ohio death certificates are public. But one part gets restricted. For the first five years after a death, the social security number is removed from the printed copy. This rule comes from Ohio Revised Code Section 3705.23. The redaction applies to all Mentor death records issued during that window. After five years, the full record is available to anyone.
If you need the social security number within that five-year span, you must prove you are an authorized person. The law lists who qualifies: a spouse, a direct descendant, the executor of the estate, an attorney, a funeral director, a government investigator, a licensed private investigator, or a veteran's service officer. You submit documentation along with the standard application and a non-redacted request form. The Lake County office will review it before printing the full copy.

The state fee is $21.50 per copy as of January 2025 under Ohio Revised Code Section 3705.24.
Note: Death records more than 50 years old may be moved from the Ohio Department of Health to the Ohio History Connection for permanent storage.
What Mentor Death Certificates Show
A Mentor death certificate lists the full name of the deceased, date of birth, date and place of death, cause of death, and manner of death. It shows the parents' names, marital status, usual address, and the attending physician or coroner who signed it. Burial or cremation details appear as well. Under Ohio Revised Code Section 3705.16, every death in Ohio must be registered with the local registrar. The funeral director gathers the personal facts and files the certificate with the county.
People use death certificates for many things. You might need one to settle an estate, close a bank account, file an insurance claim, or transfer property. Courts accept a certified death certificate as proof of the facts it contains. If you are doing genealogy work, older Mentor death records can fill in dates and family names that are hard to find elsewhere.
