Youngstown Death Certificate Office

The Youngstown City Health District - Vital Statistics Division is at 9 West Front Street, Suite 200, Youngstown, OH 44503. Phone: 330-742-8212. Fax: 330-742-8228. Email: vitalstatistics@youngstownohio.gov. The office is open Monday through Friday, 8:00 AM to 3:30 PM.

This office serves as the primary registration district for all of Mahoning County. That means death records for Youngstown, Boardman, Austintown, Canfield, and the rest of the county are all here. The one exception is the city of Struthers, which kept its own death records from 1909 through 1932. For those specific years, contact the Struthers Board of Health at 330-755-7847.

Death records at this office go back to 1892. That is 16 years before most Ohio counties started recording deaths. Birth records run from 1908 to the present. Walk-in service does not need an appointment. Bring a valid photo ID.

Getting Youngstown Death Certificates

There are four ways to get a Youngstown death certificate. The cost is $25.00 per certified copy no matter which method you pick.

In person is the fastest. Go to 9 West Front Street, Suite 200 during office hours. Certificates are usually ready in about 15 minutes. Payment options include cash, credit or debit card (with an extra fee), money order, or cashier's check. Personal checks are not accepted. This is the best option if you need the certificate the same day.

By mail, download and fill out the Application for Certified Copies from the city website. Make your money order or cashier's check payable to "City of Youngstown." Include a self-addressed stamped envelope. Mail everything to Vital Statistics Division, 9 W. Front St., Ste. 200, Youngstown, OH 44503. Processing takes a few business days after the office gets your request.

Online through VitalChek is the easiest way to order from home. Phone orders also go through VitalChek at 866-277-9849. They charge a processing fee of $10 to $15 on top of the $25.00 base cost. Youngstown Death Records and SSN

Social security numbers on Youngstown death certificates are redacted for the first five years after death. This is the standard rule across Ohio under Ohio Revised Code Section 3705.23.

To get an unredacted copy with the full SSN, you need to be on the authorized list. That includes a spouse, descendant, executor, attorney, funeral director, law enforcement officer, government investigator, or veteran's service officer. Bring your ID and documentation that shows your relationship to the deceased. The staff will verify your status before printing the unredacted version. Death certificates are only issued for deaths that took place in Mahoning County. Birth certificates, on the other hand, can be issued for anyone born anywhere in Ohio.

The Mahoning County Probate Court holds older records. The courthouse is at 120 Market Street, Youngstown, OH 44503. Phone: 330-740-2314. They have birth and death records from 1867 through December 19, 1908. Marriage records go back to 1846, and probate records start from the same year.

These pre-1908 records are ledger entries, not the printed certificates you get today. The WPA created index cards for them in the 1930s. The original records for 1846 through about 1900 are stored at the Mahoning Valley Historical Society Archival Library at the Arms Museum. This is a good resource for genealogy work in the Youngstown area.

The Mahoning County Board of Health at 50 Westchester Drive, Youngstown, OH 44515 also plays a role in the county health system. Their number is (330) 270-2855. The Ohio History Connection holds state copies of death certificates from the early filing years and has a searchable online index.

Ohio Law and Youngstown Death Records

Youngstown death certificates are public records. Ohio Revised Code Section 149.43 makes that clear. Anyone can request a copy. You do not need to be family. You do not need a reason.

Under Section 3705.16, every death must be registered with the local registrar. In Mahoning County, that means the Youngstown City Health District. The funeral director fills in the personal information and files the paperwork. A doctor or coroner signs off on the cause of death. Section 3705.07 requires the registrar to number each certificate, keep a local copy, and send the original to the state office in Columbus.
