Warren County SSN Redaction Rules
Starting October 15, 2015, Warren County began hiding the social security number on all death records filed within the last five years. This matches the statewide rule under Section 3705.23. If you need the SSN on a death certificate, you must complete a Proof of Relationship Verification Addendum. You also need to show a photo ID and proof that you qualify as an authorized requestor.
Authorized requestors include the spouse, a direct descendant, the executor, an attorney, a funeral director, a government representative, a private investigator, a veteran's service officer, or accredited media. If the proof you submit is questionable, the office sends it to the Ohio Department of Health for approval. That can cause a delay, so make sure your documents are clear and complete.

Death Records Under Ohio Law
Section 3705.16 of the Ohio Revised Code requires every death in Ohio to be registered with the local registrar. The funeral director files the certificate. The physician or coroner fills in the medical section within 48 hours. The local registrar in Warren County numbers the certificate, keeps a copy, and sends the original to the state per Section 3705.07. Falsifying a vital record is a crime under Section 3705.29.
