What Butler County Death Certificates Show

A certified copy lists the full name of the deceased, date and place of death, cause and manner of death, attending physician or coroner, date of birth, parents' names, marital status, usual residence, and social security number. The SSN is hidden for five years after death under Section 3705.23 unless requested by an authorized person such as a spouse, descendant, or estate executor.

Under Section 3705.16, all deaths must be registered with the local registrar. The funeral director files the certificate. The medical section must be completed within 48 hours. Burial permits cost $10.00 and require a filed certificate per Section 3705.17. Altering a death certificate is a criminal offense under Section 3705.29.
