Lakewood runs its own vital statistics division. This is different from most Cuyahoga County cities, which use the Cleveland Department of Public Health or the Cuyahoga County Board of Health for death certificates. The Lakewood Division of Vital Statistics is the place to go for any death that happened within city limits.

The office is at 12805 Detroit Avenue, Lakewood, OH 44107. Phone is (216) 529-7690. Walk-ins are welcome and no appointment is needed. You can pay with cash, check, money order, or credit and debit cards. But there is a catch with cards. You must show ID if you pay by card or check. Apple Pay and tap payments are not accepted. A certified death certificate costs $25.00.

Order Lakewood Death Records by Mail

To get Lakewood death records by mail, download the fillable application from the city website. You need to open it in Adobe Reader for the fill-in fields to work right. Complete the form, include a check or money order for $25.00 made out to City of Lakewood, and add a self-addressed stamped envelope. Mail it to City of Lakewood, Division of Vital Statistics, 12805 Detroit Avenue, Lakewood, OH 44107.

You can also email the completed form to vitalstats@lakewoodoh.gov or fax it to (216) 529-5910. Here is the key part: no certificates go out by email or fax. A staff member will call you back to take your payment over the phone. Then they either mail the certificate or hold it for pickup. This is a good option if you cannot get to the office and do not want to mail a check.

Under Ohio Revised Code Section 3705.23, a certified copy has the same legal weight as the original record. The copy includes the registrar's seal, signature, and issue date. Courts and banks accept it as proof of death for estate matters, insurance claims, and property transfers.

Lakewood and Cuyahoga County Death Records

Cuyahoga County is different from most Ohio counties when it comes to death records. Several cities in the county keep their own vital statistics offices instead of using a single county office. Lakewood is one of them. The others include Bedford, East Cleveland, Parma, Rocky River, and Shaker Heights. Cleveland's Department of Public Health handles the rest of the county. If you are not sure where a death took place, start with Lakewood's office for deaths in the city, or call Cleveland at 216-664-2315 for deaths elsewhere in the county.

The Cuyahoga County Archives at 3951 Perkins Avenue in Cleveland holds historical records including death records from 1840 to 1908. The Cuyahoga County Probate Court at 1 West Lakeside Avenue also has death records from 1868 to 1908 on microfilm. These are useful if you need a Lakewood area death record from before the modern vital statistics system started.
