The Carroll County General Health District handles death certificates for the county. The office is at 301 Moody Ave SW, Carrollton, OH 44615. Call 330-627-4866 ext 1557 for questions. Certificates are issued Monday through Friday from 8:00 AM to 4:00 PM. The health district only issues death certificates for deaths that happened in Carroll County. If the death occurred elsewhere in Ohio, contact that county's health department or the state.

The fee is $27.00 per certified copy. Walk-in requests are the fastest way to get a certificate. Bring the full name of the deceased and the date of death. Staff will search the records and print a copy while you wait. The office has records from 1908 to the present. New death certificates take a few days to enter the system after a death occurs.

Requesting Carroll County Death Certificates

You have three options. Walk in at 301 Moody Ave SW in Carrollton during business hours. Bring the name, date of death, and payment. Staff can pull the record in minutes. Mail your request with $27.00 per copy to the Carroll County General Health District at the same address. Include your return address and a completed application form.

Online ordering is available through the Permitium system. This is different from VitalChek, which many other Ohio counties use. Permitium handles the digital payment and processing. Death certificates are public records under Ohio Revised Code Section 149.43. Anyone can request a copy. The social security number is redacted for the first five years after death per Section 3705.23. To get an unredacted copy during that period, you must be an authorized person such as a spouse, descendant, executor, funeral director, or law enforcement.

Note: New birth certificates at the Carroll County health district take up to two weeks to become available after the birth event, so a similar delay may apply to newly filed death certificates.

Carroll County Death Record Fees

Certified copies cost $27.00 each at the Carroll County health district. Some older listings show a lower price of $23.00, but the current fee is $27.00. Online orders through Permitium may have additional service charges on top of the base fee.

The Ohio Department of Health charges $21.50 plus a $5.00 modernization surcharge for state-level orders. Ohio Revised Code Section 3705.24 says the minimum search fee cannot be less than $12.00. A portion of the fee goes to the state vital statistics office each quarter.

Death records from before 1908 are not at the health district. The Carroll County Genealogical Society holds those older records. The society is at 70 2nd St. N.E., Carrollton, OH 44615. Call 330-627-9411. Records from 1867 to 1908 are available there. These predate Ohio's statewide vital records system.

State-level historical records are also available. The Ohio History Connection has death records from 1908 through 1970 with a free online index. The Ohio Department of Health holds records from 1954 to the present. Under Section 3705.07, local registrars send original death certificates to the state. The state maintains a permanent index of all Ohio deaths, which helps when you are not sure which county a death occurred in.

What Carroll County Death Certificates Show

A Carroll County death certificate shows the full legal name of the deceased, date and place of death, cause and manner of death, and the certifying physician or coroner. Additional details include date of birth, parents' names, marital status, usual address, and social security number. Under Section 3705.16, every death in Ohio must be registered with the local registrar.

The funeral director files the certificate and gathers personal information from the family. The medical section is completed by a doctor or coroner within 48 hours. A burial permit costs $10.00 and needs a filed death certificate per Section 3705.17. Altering or forging any vital record is a crime under Section 3705.29. Death certificates are accepted by courts as prima facie evidence of the facts they contain.
